| First Day of Exclusion Policy | |

The “First Day of Exclusion” policy will apply to ALL NEW students including freshmen. This exclusion will also apply to all students who have been notified of delinquent immunizations. a current Certificate of Health Examination and Immunization Form MUST be on file in the Health Office and/or updated before the first day of school (August 20,2010). This policy means that a current Certificate of Health Examination and Immunization Form MUST be on file in the Health Office and/or updated before the first day of school (August 20, 2010). All Freshman physicals have to be in the Health Office by August 2, 2010. Athletics begin on August 11, 2010. Freshman Athletes need to turn their physical examination forms into the Health Office. The Athletic Office will not accept freshman physicals. Please be sure not to confuse the athletic forms with those of the Health Office. All athletes need health examinations and immunizations before the first day of practice or tryouts. No Exceptions (Valid within one year of the entire period of participation). All freshmen must have an Illinois Department of Public Health Child Examination form on file. These copies are kept at Lake Forest High School for 60 years in a safe place. It is for your student’s benefit to have these done correctly. Extra copies are available in the Health Office or the Counseling Office. It is parents' responsibility to inform and update the Health Office regarding students' medical needs. If your child has medical needs that may need attention during the school day, including allergy emergency care e.g., Epi Pen, please contact Diane Bower RN to ensure all paperwork and medicines are in order.
