On July 28, 2011, Governor Quinn signed HB 200 in conjunction with the
Illinois High School Association requiring educational materials as well
as ALL parents and athletes to sign off on concussion awareness PRIOR
TO PRACTICE and PARTICIPATION in any sport. The form and additional
